Description: Want to allow supervisors a standardized way to submit a report if an employee is absent or tardy

Figma Design:

**Acceptance Criteria: **

Options for reason for report MC: Tardy, Absent
Options for if they gave reasonable notice: Yes, No
Supervisors should not be allowed to submit the report modal unless the two dropdown options are completed (reasoning is optional)
