original_text is frequently missing from output

Hovering over a claim, it’s common to see “not found”, and clicking on the claim results in a large block being highlighted (rather than a single sentence):

This happens when the original_text field is not included in the model’s output.

I wonder if this happens because none of the example outputs fed to the prompt include original_text?
