MySQL 8.0 removed my_bool #5

MySQL 8.0 removed my_bool. I got mine to compile and (quickly) tested as working by just using bool.

Also, I had to update the Makefile

CFLAGS := -shared -fPIC -I/usr/include/mysql

to get it to find mysql.h. But that may have been just my system, I can't be sure.
