Automatically pull start date for comorbidities and make it not required.

From meeting with Arden on 8/16/2016. Arden noticed that 'Start date' was a required field for 'Conditions'. She mentioned that this would likely be too burdensome to manually abstract. She asked if it could be automatically populated for entries pulled from the EDW but left blank for entries manually entered. Like for patients form the private urology group.
