Shadow Reflection not recorded correctly

During a game, I played a Jade Swarmer and then played the Shadow Reflection of Jade Swarmer. Both were killed under my control--not Potion of Madness'ed or Polymorphed/Hexed. Yet under N'Zoth, Graveyard only listed a single Jade Swarmer as having died. If it helps at all, my deck has two Jade Swarmers and by the end of the game, I had not yet drawn the second one.

Wanted to add that I have confirmed that N'Zoth does reincarnate deathrattle minions that were produced by Shadow Reflection.

Unfortunately I do not have Valeera the Hollow and so diagnosing this issue would be very difficult.
