Sucessfully did a sync in AntennaPod using local ip addess (so no domain) of PodFetch docker instance.Sync completes with a sucess message. However nothing appears inside the app on the Home or Subscriptions page. I have several podcasts in PodFetch.
Reproduction
Setup new instance of Podfetch.
Subscribed to Podcast with RSS feed.
Connect to local IP of Podfetch container in AntennaPod

That is expected behaviour. I had to set them up separately. If another user wants to connect to AntennaPod he/she would download everything that the administrator downloads to PodFetch. That is why PodFetch and AntennaPod podcasts are separate identities. You can download them independently. But if the listening history matches it syncs in both ways.
This enables all users to have different podcasts on their phones but still sync to PodFetch.
Ok. There doesn't seem to be any documentation that explains this either on this repo or the AntennaPod repo unless I missed it (which is very possible). If there isn't anything written my suggestion would to be to add something to the bottom of this page
