Make appeals continuous vs. independent (Accounts only)

We have so many appeals that it's hard to follow the laundry list of existing appeals.

Things to think about:

• Design visual flow for process
• Security: Ensure that only verified appeals can see the other appeals
• Ensure IP appeals are not linked (unless part of Enable emails and allow onwiki account verification #438 )
• Implement a go into appeal
• Implement a come back out of appeal
• Make viewable from the admin side
• Possible: Address multiple NOTFOUND appeals by pointing them more clearly to what needs to be changed
• Resolve Add ban notice to top of appeals if active ban #238
