If you have 'show avatar' selected but there is the placeholder showing, I think showing a placeholder avatar probably makes sense otherwise it looks a little unexpected:

Chiming in that this is still an issue while doing QA for 6.1-beta2 with the twentytwentythree theme, and without Gutenberg installed.

That means that the current functionality will soon to be shipped with WP Core.
