Recommended template for reponse to reviewers in resubmission?

Would it be worth having a suggested template, or an example to demo how to structure the document to respond to each reviewer concern clearly and concisely? Perhaps something like this sample.

We would very much welcome you to make such a thing. We'd need a LaTex template and a Word equivalent.

After reading now several responses (as AC and reviewer), they were all different but that was definitely OK! Each author wants to emphasize different things, so I would let them have some freedom (as in journals).
