Jira's bot (https://github.com/apps/jira) will automatically parse issue keys into links if surrounded by brackets

I've been using it and it's been really helpful! :)
Description
If enabled, Jira's bot will update PRs with a link to the issue/story. It only needs the key to be surrounded by brackets!
This PR updates the commit template with those brackets.
In the case of a single-commit PR, the template will populate the PR body, which in turn will be scanned by the bot.
I'm also updating the README to reflect this change.
