I copied the memory card image on a third party memory card which has a 128KB EEPROM (checked the serial on it) using Memory Card Annihilator v2 on a SCPH-77004 slim PS2. When I boot the PlayStation (lid open and MC on correct slot) and select "MEMORY CARD" it just opens the memory card manager. Tried both the slot .mcd images.

Discovered the PlayStation has a modchip. (Didn't notice it because the laser is at the end of its lifespan and doesn't always read disc. Since it wasn't booting a backup CD I assumed it wasn't modified.) I don't know if this makes a difference.
