Time out warning and screen

What problem does this design solve?
Help users to stop their application/use of a logged in service from timing out. Explain to users why the service has timed out and what to do next.

How have you approached the design?
Based on GDS pattern discussed in alphagov/govuk-design-system-backlog#104

Session time out is set at 20 mins per page. Warning is triggered after no action for 15 minutes. Warning counts down for 5 minutes and then session times out. User is navigated to session timeout page and told they need to start application again.
