There is an error in the README. There is an example image showing using terminal-notifier for a confirmation question, but the example code next to the image is for a custom icon, not for a confirmation message.
If this is what is meant by "action buttons", then the wrong image was probably removed from the README when that feature was removed from terminal-notifier.

@brandondrew, I am terminal-notifier user not a maintainer but I think that my hint may be helpful for you and for other users. You can still use terminal-notifier for (basic) sticky notifications as for any other app on macOS showing notifications. For this you need to go Settings->Notifications, locate terminal-notifier in the list of the apps set alert style from Banners to Alerts and you have it. Unlike the image in the README, the buttons will have "Show" and "Close" labels.
[alerter] is a way for customizing sticky notifications.
