Remove or Fix Provisioner Name for Driver #116

Provisioner name and driver name should match, they currently do not. Apparently provisioner name can just be removed and it will be automatically populated as driver name.

The provisioner that can be removed is the flag to the external provisioner, however that has not been implemented.
Must wait until this is resolved: kubernetes-csi/external-provisioner#109 (comment)
