Running heads do not work as advertised

I'll need to take that back. ;-) A bit fuzzy since I can't go back and test this very carefully.

The documentation for running heads is not all accurate relative to how they really function. They seem to be semi-automatic (if you are already setup for a Table of Contents?) but I seem to recall not being 100% satisfied even than. I think the documentation suggests you author your own content (just once?).
