Add cache headers

Currently cache headers aren't sent on http responses. Commit f6ee6ce hacks around this by telling varnish not to cache these requests, however this isn't ideal as:

• It disables caching of everything (including resources which change less often).
• It doesn't affect any caches beyond varnish
