Intake - Minor UX issues

• Weight last updated has text: Weight last updated: N/A
• Text on primary button is grey
• Policy holder date of birth only goes to 1999
• Date of birth input width is too small, it should use the whole row
• Footer: Exclamation mark color and placement is wrong
• Patient sex options are different in some pages, remove them from "Patient Details" screen
• HIPAA Acknowledgement and Consent links are faulty
• Users can add the same medication twice on the “Current Medications” page. Same problem with “Allergies”, “Medical History”, and Surgeries” pages.
