EHR - Loading appointments fails

In some cases get-appointments zambda returns 500 and cannot load the appointments

When does this happen?

Selected location for the sample appointments was being stored in the local storage, which could potentially cause an issue when locations are updated and previously selected location was deleted.

Another issue with get-appointments zambda was phone numbers from sample visits couldn't pass formatPhoneNumber() function for validation, consequently returning 500.
