Usage survey settings refuse to save #305

Bug description

State of usage survey settings appear to not persist

Steps to reproduce

  1. goto /settings/admin/survey_client
  2. uncheck everything and hit "Send new report now"
  3. reload page

Expected behavior

previous selection should be saved, instead every box is ticked again so I can seemingly not deactivate usage surveys

You can always disable the App completely.
This does not fix the problem of the checkboxes (I've not tried to reproduce it) but at least you disable usage survey reports.

I'm less concerned about it collecting such data but since this is bundled, enabled, and shipped in AIO (maybe also on other install methods?), this should be fixed before people jump to the wrong conclusions :)

I was able to reproduce it on 2 more AIO instances.
