Clarify "External Mutation ID" and rename

We currently have a catch-all column for External Mutation ID to capture MGI ids or other animal model database mutation IDs. The vague naming is confusing, because we also have a human mutation ID (clinvar). We should consider more clearly scoping the data that should be contained in this attribute, and rename if necessary.
