[buildings] enhancement: render named building entrances in mapnik

I propose that named building=entrance nodes/ways be rendered at z18 in mapnik as a tiny grey label, similar to housenumbers/housenames but possibly smaller font. Osmarender already shows entrances as a tiny square at z17 + a noticeable blue label. (http://osm.org/go/0Oy6SiFWt-?layers=O "Wejcie A")

A popular use for osm is showing people how to get to your home and in the "east Germany" style giant blocks of flats the entrance/stairwell name is an important data point.

I propose to render every kind of building entrances like osmarender already does. It allows people to see to which side of the building they have to walk/drive in order to get into it.
