I have just tried to run Civilization I under Windows 11 with visual theme enabled, with no success.

This is expected behavior if the program draws the control in its own way. Also note that EnableVisualStyle is not a practical setting and can break compatibility.

The setting does not work at all: even the windows' borders and captions remain "Windows Classic"

DisableAero=1 must be set.

Disable Aero does not affect anytrhing either. The window captions are Windows Classic.

I made a mistake, DisableAero=0
