Stats should provide the name of the namespace for each item

Especially when the converted JAMS are used, the output of the stats should allow for the identification of namespace-specific vocabularies. This implies an extra field for each chord, providing the name of the corresponding namespace.

In order to make everything aligned, I regenerated all the conversion metadata files adding two columns:

• original_namespace
• converted_namespace
