Batch incremental deferred responses. Fix hanging behavior on last response

Iterate on my previous PR: #3341
Few changes:

• Write boundary right after delivering initial or incremental payload, this helps clients like Apollo Client to parse the received payload and wait on the next one.
• If the last payload doesn't have a next page, write the final delimiter, examples:

• Deliver incremental payload batched together to decrease the number of payloads sent over the wire.

This PR also works correctly with #3356

I have:
