Addressing #4244…

  • Assume a unified E axis unaffected by tool-change, and only keep one last_e_position
  • No more need for _HOTEND_EXTRUDER - dropping
  • Apply HOTEND_INDEX in MILLISECONDS_PREHEAT_TIME handling
  • Keep only a single set of PID values when HOTENDS == 1
