Support creating and editing groups of observations.

Looks like 'Family History' of cancer should supported by allowing for the creation and editing of groups of observations. For example, a group of 2 observations.

• Observation 1: to collect a hardcoded concept of 'Family history of neoplasm' as the measurement_concept_id and value_as_concept_id as the type of relation. For example, brother, sister, mother etc.
• Observation 2: to collect the age at diagnosis of the family member with cancer.

This seems to be in line with the best practice guidance within the ODHSI community;
