Clinical stage does not belong to any specific clinical event. It is an interpretation made by the data coordinator based on the DRE result, the latest PSA value and biopsy result.
Answer: Maybe make it a group of observations to capture the TNM staging (see #19):
T category: The extent of the main (primary) tumor
N category: Whether the cancer has spread to nearby lymph nodes
M category: Whether the cancer has spread (metastasized) to other parts of the body.
If the different possible combinations can be pre-coordinated, go with only one observation.
