This PR moves the document outline/content structure button back to the top toolbar, from its location in the footer where it resided as part of my efforts in #19348. It also received a new icon:
It was moved to the footer to reduce the weight of the top toolbar buttons, where only the most important buttons should sit. However the presence in the footer buries the feature, which deserves better.
I have also created #20719 to revisit this toolbar balance.
