Description

As an admin, I need to be able to create forms so that the users can fill them. Admin should also be able to view the forms from the user's perspective as well.

Mocks

As finalized in issue #6

Acceptance Criteria

The form should have :

  • The form should have all the components as the mockup
  • consume backend APIs

Definition of Done

  • All of the required items are completed.
  • Approval by 2 mentors.
