Update disclosure log

Summary

We should consider updating the disclosure log with newer problems we recently found:

• @istoilkovska reported an issue where the channel closing protocol is underspecified. If this issue can surface as a bug in an implementation, we should probably report it in the disclosure log.
• while implementing the connection handshake message processing logic (Make ClientState not optional in MsgConnectionOpen{Try,Ack} #160), we uncovered a potential problem, reported to ICS spec team: ICS3 handshake version negotiation problem ibc#459 -> moved into its own issue (Add check for redundant relays and treat them as noops (although an event must be emitted) in AcknowledgePacket and timeoutOnClose #213)
