Added alternate instructions to build the project inside a container with a host-mapped folder, then deploy to a runtime-only container.

IMO multi-step builds will make this overall experience much nicer. Since Docker is releasing this feature in stable this month we may consider holding off for this PR instead.

Sure - I'm ok either way. This is basically a deconstructed multi stage build :)

Closing as the samples have now been upgraded to make use of the Docker multi-stage build feature - #47
