Add Form 1M to the Report Selection

Business Reason

As a user, I will be able to see 'Form 1M:' "Notification of Multicandidate Status" in the Form type selection so that I can create a new Form 1M report.

Acceptance Criteria

Given I'm a user
When I select 'Create a new report'
Then the 'Select form type' popup will include "Form 1M: Notification of Multicandidate Status" in the selection dropdown, below Form 3X

QA Notes

• BA/Design Quality Checklist complete
• Form 1M selection will be grayed out until it is developed.

DEV Notes

• Form 1M selection will be grayed out until it is developed.
