A *lot* of changes...

Most notable:
* Most of the pointer fiddling is replaced by range operations. The functions to getInt etc have been replaced by consume!(T) calls.
* Rewrote row fetching to simplify the large constructor (only non-binary tested). Getting chunked values isn't implemented yet
* Forcing protocol 4.1 and long passwords
