Adds header and request settings options

These changes will allow for enabling headers and request settings for a vcl service. Both use dynamic blocks. Multiple header types can be enabled by creating multiple objects in headers = [{}]. Setting fastly_enable_header and fastly_enable_request_setting to true will enable those options and will output default values unless overwritten.
