So scraping images from https://www.mangahere.cc/ and from https://fanfox.net/ is no longer possible because of how these sites serve images, so issues related to these can be closed. Maybe even the extractors can be removed.

Image downloads from those two sites yield 403 errors, but that can be circumvented by simply sending a Referer header (3fd9249, c8abb16).

-o headers.Referer=https://www.mangahere.cc/ to make this work with the current gallery-dl release.
