Also move fathom site ids to model/fathom
Based on existing ontologies, including SKOS:Concept. The SKOS ontology will give us lots of useful predicates for noting things are related, labeled, or in a particular Collection. It will also allow us to easily specifiy notes (which can be any format, not just text documents), so we can relate these Concepts easily to both our notes, as well as Images and Links. This change has been prompted by the need to add Links and Images to a garden without having an assocaited label or name yet.

This is the first PR of this repo, and takes us from the last commit of understory-garden/understory.garden to the first commit of mysilio-co/garden. Most of the bones are the same, but there have been some major revamps to the editor, and will soon be more revamps to the design and functionality. There is too much to go into detail on all the changes here, but I wanted a single PR to explicitly denote the changes I made. This includes everything up to and including work last Friday to revert the hex dashboard. There is some additional work in progress that will be merged in a future PR to finish up Bookmarks and Uploads.
