added default GitHub issue templates for bug reports and feature requests.

Should we also address the title of bug report somehow in this template?

E.g., Write a title that clearly and concisely states the issue

@jessica-mitchell: are you happy with the current state? I think having this in the first place is more important than having it perfect. We can always improve on it later on.
