As in Microsoft Word, NVDA should be reading the formating document in OpenOffice.org such as style, font, font size, underline and other text atributes. This is very useful for a better document control.

### Summary of the issue:

- various code-fences are not formatted properly
- inconsistent indentation on lists
- inconsistent formatting of messages

### Description of how this pull request fixes the issue:

- enforce `- Example changelog message. (#123[, #321])` format using the following regex to highlight conforming messages `(.*\. \((#[0-9]+(, )?)+\)$|  -.*)`
- replace all usages of single ` with ``
- standarise indentation for second level lists to use the recommend two spaces
