Update license and copyright header #308

I would like to update winapi to be dual licensed under MIT and Apache 2.0. For the sake of consistency I would also like to update the copyright header in every file as follows.

// Copyright © 2016 winapi-rs developers
// Licensed under the Apache License, Version 2.0
// <LICENSE-APACHE or http://www.apache.org/licenses/LICENSE-2.0> or the MIT license
// <LICENSE-MIT or http://opensource.org/licenses/MIT>, at your option.
// All files in the project carrying such notice may not be copied, modified, or distributed
// except according to those terms.

To do this I need the consent of contributors to this project so please state whether you consent here. Even if you don't consent, I still need to know so I can rewrite your contribution from scratch. Thanks.

IANAL but... the only thing in this crate that could even conceivably be copyrightable is the layout of the modules (that's the only "creative" part and even that's a stretch) so this is probably not really necessary (but a good idea anyways).

@Stebalien Well the macros which are used pervasively across winapi could definitely be copyrightable. I just want to make sure I'm legally in the clear and that nobody has the right to complain when I do change the copyright/license stuff.
