Wrap the spelling plugin in a sphinx domain. Use `spelling:word-list` instead of `spelling`.
Extract the logic for managing the per-document temporary word list from the directive so it can be reused outside of the directive.
Add a role to support identifying inline text as being spelled correctly. Fixes sphinx-contrib#9
