Adoption Form

About

SWERA team collects information about farmers who adopt SWERA's mode of farming. This information is collected in a physical form called adoption form. Data collected is used for further analysis.

The team currently uses physical forms to collect data. We need to digitize the process.

Adoption Form

We need to capture following fields in a form:

• Name of farmer
• Father's/Husband's name
• Collectives name
• Village name
• Video name (this is a video though which farmers are educated)
• Date of adoption
• Area (land farmer owns)
• Date of verification
• Adoption by video or Meeting (checkbox)
