Dissemination Form

About

Literal meaning of dissemination is the opening of a subject to widespread discussion and debate. Our NGO occasionally conducts awareness programmes in villages. Lot of farmers attend these programmes to get benefit out of it. Videos of better farming practices are shown and question/queries of the farmers are answered. Few of the farmers end up enrolling in the programme to share their farming data. By following the guidelines, farmers can get better yield.

Data about each such dissemination is captured for record keeping and further analysis. Currently the NGO uses analogue format - pen and paper - to collect this data. We need to digitize the process.

Dissemination form consists of two parts -

We need to capture following fields:

• Date of dissemination
• Start time
• End time
• Place
• Name of video
• Name of Gram Panchayat
• Name of CRP
• Name of Village

2. Attendee details

We need to capture following fields:

• Farmer's name
• Husband's/Father's name
• Collectives name
• Adoption wish/Willing to adopt
• Questions asked by farmer
