Update the "Request Changes" and "Approve" buttons alignment #1350

This Pull Request updates the style of the PR Description page to move the Request Changes and Approve buttons to the right side, near the Comment button. This way, those related functionalities are grouped together and far from the Close Pull Request button.

Thanks @guilherme-gm! There's another place where buttons are put in a form-actions class, when you have a pending review:

Can you make sure all the buttons get right aligned in this case? Code is here:

Oops, I didn't notce form-actions were used in so many places. I've updated my PR to use more specific selectors and cover those 2 cases. (Images in PR description updated). Thanks @RMacfarlane!
